I hereby acknowledge service charge indicated above, a 1 hour minimimum charge per technician (for hourly services), and I authorize Dimension Systems, Inc. to proceed with work requested. As per DSI policy, there is a 30-day limit to pick-up the equipment after it has been completed. Failure to adhere to this policy will result in disposal of equipment listed above at DSI's discretion and all charges will be billed accordingly.
